Based on information you provide to the portal whenever create user account and when you begin a reporting system automatically selects the type of safety report to be submitted.
This version of the Safety Reporting Portal currently supports the following types of safety reports:
Mandatory Reportable Food report
Voluntary Reportable Food report
Mandatory Animal Drug report
Pet Food Safety report
Tobacco Product Report
Gene Transfer Research Safety report
Please note: that you will need to have a user account and
to be logged into it in order to submit certain types of reports. If you
begin such a report and have not logged in, the Portal will present you
with links to log in or create an account.
